I am using an RT-N66U with Shibby's Tomato.
I can't connect to 5 GHz on my 2013 Mac if the country is set to "UNITED KINGDOM" and Channel width set to 40 MHz (20 MHz works fine).
If I disable WPA2 or select "EUROPEAN UNION" then both 20 and 40 MHz work perfectly.
Extra info
Country set to "UNITED KINGDOM"
5 GHz only works if I set locale to "EUROPEAN UNION"
